About IVY & CO

IVY & CO Support Coordination was created with a strong focus on genuine relationships, participant wellbeing, and practical support coordination. We work alongside participants, families, and support networks to help people navigate the NDIS with confidence, clarity, and personalised support.

IVY & CO was founded by Nathan & Kodi following several years working within the disability and support sector. Through frontline experience supporting participants and families, we identified the need for more personalised, responsive, and relationship-focused Support Coordination services.

With a background in hospitality leadership, disability support work, and community services, IVY & CO was developed to provide professional support coordination while still maintaining a genuine and human approach to service delivery.

We support participants to understand and implement their NDIS plans, connect with providers, coordinate services, build capacity, navigate complex support systems, and prepare for future goals and plan reviews.
